| Initial Evaluation |
| A complete examination to determine the source
of pain and/or dysfunction is performed. This evaluation also
includes patient education, self-care, and custom home exercises
to manage your symptoms independently and decrease healing time. |

| Re-evaluation |
| During this session, re-assessments
and therapeutic progress notes are completed.
Updates to self-care, home exercises, and general treatment
regimens are performed. Objective measurements are taken to
determine if physical therapy treatment goals
are being met. Physician progress reports are sent, if needed. |

| Manual Therapy |
| Includes an assortment of hands-on
physical therapy techniques to decrease pain and tension, and/or
increase range of motion and flexibility: |

| Soft Tissue Mobilization/ Myofascial Release |
| Manual techniques are used
to inhibit pain, reduce tension, and decrease scar tissue. |
| |
| Joint Mobilization |
| Specific forces applied to
the affected joints and collagen fibers at various amplitudes
and frequencies are used to decrease pain and increase mobility.

| Other Manual Therapy |
| Neuroflossing is used to decrease
pain caused by entrapped nerves. |

| Therapeutic Exercise/Activity |
The
appropriate physical therapy exercises and activities
