For your convenience you may download the necessary forms that are required to be completed before your scheduled physical therapy appointment. Total Body provides physical therapy services in good faith and knows that its patients will do their best to provide payment for services.

The following information will provide you with general guidance  regarding our financial policies, especially as they relate to medical health insurance.

Medical Insurance
Total Body currently accepts most major PPO insurance, some HMO insurance, Medicare, Worker’s Compensation and Cash (please see below). You are responsible for understanding your insurance policy(s) and procedures. However, we will be happy to assist you in any way we can with specific questions and concerns.

Total Body is a member of the Physical Therapy Provider Network As a member of the largest insurance provider network, Total Body is able to offer you the best care physical therapy has to offer at minimum cost to you. PTPN includes over 1000 insurance providers. To determine if your insurance company is in PTPN, please call our front office during normal business hours.

Payments
We will require payment at the time of your office visit. For your convenience, we will accept cash, check, debit cards and most major credit cards. Please contact us for more information or to schedule an appointment.

You can choose your own physical therapy clinic.
We accept all physical therapy prescriptions

*Patients may incur extra cost for home care equipment, braces, etc.
